As artificial intelligence (AI) rapidly matures, its role in healthcare continues to expand—from diagnostics and triage to decision support and population-level risk modelling. Yet the success of any AI system ultimately hinges on one critical factor: public trust. A new South African study offers valuable insights into how people perceive AI in high-stakes medical situations — and these findings carry important implications for Australia, New Zealand, and the Pacific. 

A recent Swedish study has shed light on the extraordinary challenges faced by ambulance nurses during the COVID-19 pandemic, offering valuable insights into frontline decision-making, adaptation, and learning under extreme conditions. Drawing on 56 critical incidents reported by 28 registered nurses, the research identified four key themes that illustrate the complexity and ethical weight of their experiences. 

The transition from paper-based to electronic patient records (EPR) is critical in modernising healthcare systems. A recent study in Cureus explored how a UK hospital successfully adapted its existing software to create a cost-effective, clinician-friendly EPR system. The findings offer valuable insights for hospitals worldwide, including those in Australia and New Zealand, where digital transformation remains a priority. 

The Global Patient Safety Report 2024 by the World Health Organisation (WHO) highlights the critical global issue of patient safety, recognising it as a key component of quality healthcare that still requires significant evolution. The report's main goals are to raise awareness of patient safety risks, provide insights into the scope of the problem and set the stage for global initiatives to reduce harm to patients in healthcare settings.  

The inter-dependency of clinician wellbeing and patient safety is well accepted. Staff burnout is a widespread problem and patient safety improvement appears to have stalled. This survey of 2187 physicians and 6643 nurses practising in 64 hospitals in six European countries investigated the well-being of physicians and nurses in hospital practice in Europe, and identified interventions that hold promise for reducing adverse clinician outcomes and improving patient safety.

It is 10 years since Robert Francis presented the report on the public inquiry into failings at Mid Staffordshire NHS Foundation Trust. The government’s response signalled a determination for change. It promised wide-ranging interventions and legal and regulatory reforms to tackle problems of culture, openness, and willingness to learn. What is the impact 10 years on?

The concept of candour is not new. Being candid means being honest, which includes ‘complete openness and transparency.’ Exercising candour in healthcare is challenging for multiple reasons.

‘Successful integration is the planning, commissioning and delivery of co-ordinated, joined up and seamless services to support people to live healthy, independent and dignified lives and which improves outcomes for the population as a whole.  Everyone should receive the right care, in the right place, at the right time.’

The term ‘clinical governance’ was first coined in the context of the National Health Service (NHS) in the United Kingdom, being ‘a system through which NHS organisations are accountable for continuously improving the quality of their services and safeguarding high standards of care by creating an environment in which excellence in clinical care will flourish.'

From its inception as a concept, clinical governance has learned from failures.  A landmark inquiry into the deaths of over 30 children at the Bristol Royal Infirmary between 1991 and 1995 (Bristol Inquiry) was an early exemplar of how learning from errors can inform improvements to safety and quality, and hence clinical governance.
