A stronger and smarter approach to adverse event investigation

The London Protocol is a well-known framework designed to guide deep analysis of clinical incidents to improve safety. Originally developed for health services in 2000, it provides a systematic approach for investigating adverse events and near-misses by examining contributing factors across multiple domains, such as organisational processes, team dynamics, environmental conditions, and individual performance. The goal is to move beyond blaming individuals and instead uncover deeper systemic issues that may compromise safety.
